Swifts were the victims of a giant-killing, beaten 7-6 on penalties by Coagh.
Steven Park gave Coagh the lead before Stephen O'Neill redressed the balance.
O'Neill then put the Swifts ahead in extra-time but Park lobbed in a leveller.
Swifts goalkeeper Niall Morgan had a chance to score a winning spot-kick in the shoot-out himself, but missed and Coagh went through when Grant Hutchinson crashed his effort into the crossbar.
Linfield accounted for Ards 3-1, and Glentoran did likewise to the PSNI.
Peter Thompson fired the Blues ahead on two minutes.
But the Championship One leaders had a surprise in store for David Jeffrey's side when Paul McDowell levelled with a superb 25-yard free-kick.
But Brian McCaul and Jim Ervin scored to book the Blues place in the next round.
Richard Gibson and Mark Clarke had Glentoran two to the good at the break at half-time against the PSNI, with Jimmy Callacher restoring the two goal cushion after Lukasz Adamczyk pulled one back for PSNI
Glenavon enjoyed a straight-forward six goal win over Sport and Leisure and Portadown defeated Dundela 3-1 at Shamrock Park, with Joe McNeill, Sean Mackle and Darren Murray scoring for Ronnie McFall's side.
Carrick Rangers won 5-0 against Killymoon Rangers with Aaron Harmon, Daniel Kelly with two, Adam Dick and Kyle Owens on target.
An extra-time Gary Liggett goal secured Lisburn Distillery's win over Limavady United and Corey Heggarty scored a 90th minute Institute winner against Banbridge Town.
