Nominate your ITV Regions Fundraiser Hero with UTV Live

This year we are once again joining forces with the Daily Mirror to find the individual you think deserves to receive the ITV Regions Fundraiser Award with UTV Live.

We are looking for a specific kind of unsung hero. We are on the search for someone who raises funds for a great cause - no matter how big, or how small, their fundraising efforts. It could be someone who comes up with creative ideas to raise cash or maybe it's their passion and dedication that really inspire others to give.

The shortlist and selection panels will look at the amount of funds raised, the creativity and ingenuity of the Nominee's fundraising and personal endeavours of the Nominee when fundraising to make their decision.

If your nominee is a regional winner, they will be invited to attend the star-studded Pride of Britain Awards in London in October where the overall winner for the UK will be unveiled.

The ceremony is a prestigious event, celebrating the achievements of some truly remarkable and inspirational people.

How to nominate?

  • Visit: Prideofbritain.com or pick up a nomination form at Lidl stores;
  • Tell us in no more than 500 words, which category you are nominating them for, and why you think your nominee should receive an award;
  • Don't forget to leave your name, a daytime contact number and an email address for you.

If you, or the person you are nominating is under 18 years of age, we will need a name and daytime telephone number for you and your nominee's parent or legal guardian.

There is also some vital information we need about you and your nominee. All relevant details can be found in our full terms and conditions.

A full list of all the Daily Mirror's Award categories can be found here.

If you have a disability and require assistance with your application form or the information in an alternative format, please contact the Pride of Britain team directly at 0207 293 3599 or chelsey.blake@mgn.co.uk

The closing date for applications is Friday 15 August 2014 at 11:59pm.
