Nollaig O'Connor, who is from Enniskillen, had not been in touch with relatives since May 2013.
The 37-year-old had been studying music at university in Aberdeen but went to Cornwall. There were unconfirmed reports she was in Munich last August.
In a Facebook post on Friday, the family said they had received a "100% confirmed sighting" of Nollaig in mainland Europe.
A later message added: "Great news, I am with Nollaig south of Spain and she is safe. Even better she has agreed to come to Ireland with me and we leave on our journey tomorrow.
"We have been truly blessed this day."
An update on Sunday said Nollaig is now back home with the family and is "looking so well considering all she has been through".
Her sister Aine told UTV: "We're absolutely ecstatic, really relieved.
"I can't describe it. It has been really difficult not knowing what happened to her, if she was okay, if she was surviving, we just had no idea."
The news has been welcomed by Fermanagh MLA Michelle Gildernew of Sinn Féin.
She tweeted: "Great news from the O'Connor family overnight that Nollaig's on her way home. Thx to everyone who helped, esp @The_Westville Nicky Cassidy."
