The Ulsterman, who won the BMW PGA Championship victory at Wentworth on Sunday, had carded a superb 63 in the first round at Ohio but came crashing back to earth in the second.
He suffered a disastrous opening nine which started with a bogey on the opening hole and also included three consecutive double bogeys, as he went out in 43 at Muirfield Village.
It made for a 12-shot collapse on the back nine from Thursday where he had picked up two eagles, three birdies and a double bogey to record a 31.
McIlroy managed to improve on the back nine with birdies on the second and seventh and only one dropped shot in between as he ended on three-under for the day.
Meanwhile, in the Nordea Masters in Malmö, Northern Ireland's Gareth Maybin is on two under and Michael Hoey is on three over, with Victor Dubuisson leading the way on six under.
