The incident started when a bottle was thrown at the house of an off-duty Superintendent in the seaside town of Dungarvan on Thursday night.
When he went out to confront a group of young men, he was hit in the head.
Local gardaí arrived at the scene shortly afterwards.
The Irish Times Southern Correspondent Barry Roche explained: "They arrested two youths at the scene, another two fled, and one of those other youths, it appears, went through a building site and may have been injured on a pipe or railing and appears to have fallen into a river or a culvert and tragically died."
A statement from the Garda Síochána Ombudsman Commission said: "A team of GSOC investigators is in Dungarvan since the early hours of this morning and the investigation is underway."
