It is thought the 47-year-old driver of a silver Audi suffered a heart attack and lost control of the car along the Woodstock Road on Thursday, driving onto the footpath hitting a woman on a mobility scooter.
He was taken to hospital but died.
The woman, who was talking to two other people at a bus stop at the time, was thrown from her scooter.
The collision happened around midday near a charity shop which had its front window smashed by the impact.
A police spokeswoman said: "The death is not thought to be as a result of the collision but due to a medical condition."
John McPoland from the Northern Ireland Ambulance Service told UTV his crews were on the scene quickly.
He said: "We arrived at the scene very, very quickly with three crews and another one, three officers at the scene, one of our doctors arrived and looked after him quickly, assessed the scene and took the driver of the car to the RVH and the other people have also gone to hospital."
Despite one of the pedestrians being flung through the window of a charity shop, all three are believed to be in a stable condition in hospital with non-life threatening injuries.
Mr McPoland continued: "The other three people involved in the incident - one has suffered a head injury and the others have lower limb injuries, hopefully not life threatening."
Many people were shocked by what they saw.
Local business owner Michael Chandler said: "There was a lot of screaming, a lot of children about, a lot of people running to help the injured and you could tell people were hurt.
"Smoke or steam coming from the car and airbags were out so it looked very, very serious.
"People were doing their best to help. This road is very busy, it never stops and sometimes it gets too busy, a lot of lorries and buses and it gets very congested at times. Obviously everyone knows everybody around here. There was a huge amount of people around the injured so we didn't go too close just wanted to give them a bit of space obviously."
Part of the road was closed while emergency crews dealt with the incident.
Police in Strandtown are appealing for anyone who witnessed the crash to come forward.
